Fisher and Frey provide a a great resource for different methods for using assessment to inform teaching.  The methods range from the simple (talking with students and asking good questions) to the more complex (using projects and performances to assess understanding).  Throughout the text there are methods that can be applied to the classroom very easily and the explanation of the techniques is clear and easy to follow with several examples accompanying the text.  One resource that jumped out is the extensive collection of graphic organizers that could be utilized during formative assessments.
